New Patient Information

New patients at St. Matthew’s Dentistry can expect a high level of care in a down-to-earth local setting. In order to get started with our practice, there are a few important things you’ll need to complete prior to our first appointment and some information to keep in mind.

Paperwork & Registration

In order to give you the best possible care, it’s important for us to get some information from you about you including personal information, information about your dental and medical history. It’s also important for us to provide you with information about what you can expect from us, how we handle your information in a safe and secure way, and other details about treatment. We’ll collect this information from you during the registration process when you first arrive.

Since it’s essential to complete the paperwork before we can provide you with treatment, it’s recommended that you download and complete the forms at home and bring them to your first appointment.

Payment & Insurance

We’ll cover information about payment and insurance during our registration process when you arrive. If you will be using insurance benefits, we will make a copy of your insurance documents and cards for our records. Our staff will work with you to coordinate the necessary financial arrangements.

St. Matthews Dentistry is in-network with many insurance plans. For the most updated records, be sure to call our office to verify our participation in your plan. We’ll gladly provide you with estimated costs for services based on your insurance benefits.

At our office, we are able to accept cash, checks, and major credit cards.

Exam and X-Rays

Two major components of your first visit will likely be a comprehensive visual exam of your teeth and mouth and complete set of dental x-rays. Both of these are essential to allowing your dentist to have a good understanding of your overall oral health and to provide the necessary information for recommending treatment. Our digital x-rays are safe and provide instant imagery to our staff. This allows us to quickly and easily share the results with you.
